The 31-year-old Ballack married Simone Lambe at Schloss Kempfenhausen, a country house in Berg, southwest of Munich.
Mayor Rupert Monn officiated at the ceremony, Monn's office said.
Ballack met Lambe during his time at Kaiserslautern. The couple already has three children - Louis, Emilio and Jordi.
The Chelsea midfielder announced before the European Championship that they planned to get married.
