Fognini was punished with a penalty point in the fifth set after throwing his racket in frustration following a forehand that went wide. The racket landed near a ball boy.
The penalty point cost the 14th-seeded Italian the game, putting him behind 3-0.
Four years ago at Roland Garros, Fognini rallied from a two-set deficit to beat Monfils in the second round.
